Child Health and Disability Prevention (CHDP)

C​hild Health and Disability Prevention (CHDP) is a preventive program that delivers periodic health assessments/well-child exams and services to low income children and youth in California.  CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services.   

Who is Eligible?

Children and youth up to age 21 who are eligible for Medi-Cal. Children and youth under age 19 with family incomes less than or equal to 200% of Federal Income Guidelines are also eligible. Proof of residence and income is not required.

What is a Well-Child Exam?

This is a health assessment that consists of:

  • Health History
  • Physical Exam
  • Developmental Assessment
  • Nutritional Assessment
  • Dental Assessment
  • Vision and Hearing Test
  • Tuberculin Test
  • Immunizations
  • Health Education/ Anticipatory Guidance
  • Laboratory Testing (anemia, blood lead, etc.)
  • Referrals (as needed)
    • Providers may make referrals to the Women, Infants and Children (WIC) Nutrition Program, dentists, medical specialists, and mental health providers.

Who Offers Well-Child Exams?

Health assessments are provided by enrolled private physicians, community clinics, managed care plans, and some local school districts.
