Child Health and Disability Prevention (CHDP)
Child Health and Disability Prevention (CHDP) is a preventive program that delivers periodic health assessments/well-child exams and services to low income children and youth in California. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services.
Effective July 1, 2024 the Department of Health Care Services (DHCS) is transitioning the Child Health and Disability Prevention (CHDP) Program to other existing Medi-Cal delivery systems.
Transitioning the CHDP Program aligns with the Department’s goal under CalAIM to reduce administrative complexities, enhance coordination of care and whole person care approach, and increase standardization of care across Medi-Cal by consolidating care responsibilities for children/youth under the Medi-Cal managed care plans.
