Immunization Program
Immunization Clinics
The Siskiyou County Public Health Immunization Program offers immunizations to community members at monthly immunization clinics in Yreka and Mt.Shasta. Vaccinations against polio, diphtheria, tetanus, measles, mumps, rubella, pertussis (whooping cough), pneumonia, hepatitis A&B, meningitis, human papilloma virus (HPV), chicken pox, shingles and flu are available. Travel vaccinations are also provided when available.
- Children are encouraged to see their primary care providers for immunizations.
- Vaccines provided through the Vaccines for Children (VFC) Program and the Vaccines for Adults (VFA/317) Program are available for $9.00, or at no cost, to eligible children and adults.
- Children covered with private health insurance are not eligible to receive immunization services through these programs. Please contact your primary medical provider to obtain vaccinations.
- Some vaccines are available for retail cost, plus an administration fee, to individuals who are not eligible for the VFC or VFA/317 Programs.

- If there is a fee, payment is expected at time of service. Exact change or check is preferred. We are unable to accept credit/debit cards.
- Persons eligible for State supplied vaccine will not be denied those vaccines for inability to pay.
- We do not bill private insurance. A receipt will be issued which may be used to submit for reimbursement from private insurance.
- Please bring any prior immunization records to your appointment.
- A parent of legal guardian must be present in order for services to be provided to children under the age of 18. If a parent or legal guardian cannot accompany the minor, a consent form must be completed.

California School Immunization Requirements
California law requires all children enrolled in state schools, both public and private, to have certain doctor-recommended immunizations, or receive them when they enroll.
- Pre-Kindergarten (any private or public child care center, day nursery, nursery school, family day care home, or development center) requirements
- K-12th Grade (including transitional Kindergarten) requirements

Digital Vaccine Records
The California Department of Public Health (CDPH) has expanded the digital COVID vaccine record portal to include all vaccines, thereby providing digital access to one’s immunization record from the California Immunization Registry (CAIR). This new Digital Vaccine Record (DVR) portal provides a much-desired service, giving California residents easy access to their immunization history.
