Please fill out the form to the best of your knowledge and provide contact information. If no contact information is provided then the report will be rejected. This form is not an anonymous reporting system. NOTE: YOU MUST PROVIDE A VALID E-MAIL ADDRESS.

Only cases within the jurisdiction of the Siskiyou County Sheriff's Office will be assigned a case number. If the crime happened in another jurisdiction you must contact that law enforcement agency.  For drug related information, please call the Siskiyou Unified Major Investigation Team (SUMIT) at 842-8830.

Please do not print and mail or fax the report to the Siskiyou County Sheriff's Office. Please do not provide Social Security Numbers, Driver's License, or other private information. A Deputy Sheriff will contact you for this information.

After you submit your report it will be reviewed by the Siskiyou County Sheriff's Office. If a case is assigned to your report then a Deputy Sheriff will contact you and provide your case number.
